01 Overview of National Health Insurance

Korea implements a health insurance system wherein a personpays a certain monthly premium according to his/her income andassets.
Subscribers to the national health insurance can access medicalservices at a low price if they are sick, give birth, or simply wantto have a regular medical check-up. All citizens are required tosubscribe to the national health insurance, with the exception ofbeneficiaries of medical aid. Subscribers to the national healthinsurance system can be categorized into local subscribers andemployee subscribers. All employees of businesses, employers,public officers and teachers are categorized as employeesubscribers. Those whose livelihood depends on the incomeof employee subscribers may be registered as the subscribers'dependents if meeting the dependence requirements and incomerequirements as set forth in the Enforcement Decree of NationalHealth. Other subscribers and the family members supported byemployee subscribers are categorized as local subscribers.
For company subscribers, the company pays for 50% of the medical insurance fee (applies onlyfor salaries taken from company), and individuals have to pay the remaining 50%. Individuals whohave separate income exceeding 34 million won must pay the full insurance fee with 100% of theinsurance rate applied, after 34 million won is deducted from the total income.
When a health insurance policyholder receives treatment from a hospital, the National HealthInsurance Corporation pays a portion of the medical fee. The policyholder can receive treatmentfrom hospitals, medical centers, or oriental medical clinics. However, the other portion must be paidby the patient. There are also medical checkup benefits. The number of checkups provided will differdepending on age. Most people with insurance have a medical checkup every other year.

02 Foreign Residents Who Are Required to
Subscribe to the National Health Insurance

Foreign residents who are registered and who work at businesses covered by employee subscriber policies, or foreign residents appointed as public officers and teachers, are granted the status of employee subscribers. Foreign residents who are registered and foreigners covered by attached Table 9 of the enforcement decree of the National Health Insurance Act Insurance, foreign residents who are registered and foreigners covered by attached Table 9 of the enforcement decree of the National Health Insurance Act and who are not categorized as employee subscriber or dependents of employee subscribers may gain the status of local subscribers after having lived in Korea for six months or longer (immediately after entering Korea for marriage immigrants or qualification to stay as an exchange student).
However, a foreign resident who receives medical security equivalent to the National Health Insurance through the laws of his/her country or contracts with a foreign insurance provider may be exempted from subscription to the national health insurance upon request.

03 How to Register

(1) When Your Husband Is an Insured Employee

You may register as a dependent* on the spouse’s National Health Insurance. Submit documents confirming dependency to the National Health Insurance Corporation.

  • Required documents: A certificate listing you as your husband/wife’s dependent, a copy of youralien registration card, and a certificate of family relations (family relations certificate from a foreigncountry must be a document confirmed by the ministry of foreign affairs of the country (or Apostille)(including Korean translation))

(2) When a Married Immigrant Woman Is Employed

  • The company must submit a copy of the worker’s alien registration card and other necessarydocuments to the National Health Insurance Corporation.

(3) When Both the Husband and the Married Immigrant Woman Are Unemployed

  • Effective July 16, 2019, foreign residents who are not categorized as employee subscriber or dependent of an employee subscriber will be granted the status of subscriber of health insurance after having lived in Korea for six months or longer as prescribed in attached Table 9 of the Enforcement Decree of the National Health Insurance Act (immediately after entering Korea for marriage immigrants or qualification to stay as an exchange student). If foreigners are residing in Seoul, Incheon, and Gyeonggi area, they should visit the foreigner center below to acquire qualification, combine households, and pay insurance.

  • Foreigners residing in Gwacheon, UIwang, Hanam, Gwangju, Yangpyeong, Icheon, Anseong, and Pyeongtaek (including Korean national abroad) should visit a branch office of jurisdiction for processing.

04 Payment of Insurance Premium

(1) Employee Subscribers

Payment of Premium :
Monthly premiums are deducted from monthly salary and are remitted by the employers.
  • As for employee subscribers, monthly premiums are deducted from monthly salary (50% being paidby the employee and 50% by the employer), and monthly income premiums should be paid byindividuals in relation to their gross annual income (in case of exceeding KRW 34 million).

(2) Local Subscribers

Payment of Premium
  • For foreigners, the monthly insurance fee must be paid one month in advance by the 25th of the previous month (the insurance fee incurred by the acquisition of retroactivity is added to the first payment). When the calculated insurance fee is less than the average insurance fee of the previous year, an average of the total of all subscribers’ fees is charged. The insurance fee in 2021 is KRW 131,790.
  • Permanent resident foreigners with F-5 or F-6 status of sojourn pay the same insurance premium as Korean nationals on the 10th day of the following month.
Note

When a foreign spouse living with a Korean citizen is listed in Korea’s health insurance card, the subscriber pays the amount calculated by the same standards applied to Korean citizens.

  • When the relationship with the Korean citizen is verified, other subscribers may join the household.
  • In the case of a household consisting only of minors below 19 years, the same insurance premium as Korean nationals will be applied.

05 Penalties for Failure in Payments

  • When a foreign local subscriber fails to pay the insurance fee by the 25th of the previous month, insurance benefits will be restricted from the 1st of the following month until the overdue amount is paid in full.
  • When the insurance fee or other health insurance collection is overdue, visa extension by the Ministry of Justice is allowed up to six months only. If late payment occurs three times, extension of stay may be restricted.
  • In the case of overdue amounts of KRW 500,000 or more of insurance fee and other collection fees of KRW 100,000 or more, the history of late payments will be reflected during the stay permit (registration) screening by the Ministry of Justice.
